Eliminate double-invoke leak in the Offscreen cache

The brief

Offscreen boundaries in hide/show loops can double-invoke effects and leak retained fibers. Plug the leak without changing visible scheduling semantics.

Acceptance criteria

• Loop repro shows effects fire exactly once per commit
• No new act warnings across the Offscreen fixtures
• Memory profile flat across 1,000 hide/show cycles
